The physical location where the healthcare service was rendered to the patient. This appears in Box 32 of the CMS-1500 claim form.

The service address tells the payer exactly where the patient received care. This is critical for:

What is a Billing Address?

Billing Address (Pay-To Address)

The billing address is used for:

  • Correspondence regarding claims
